Client Rights and Boundaries

 

As a client, you have the right to:

  • Safety and Respect: To be treated with dignity, compassion, and respect at all times.
  • Confidentiality: To have your personal information and anything shared during sessions kept private, except where required by law.
  • Choice and Autonomy: To choose what you share, to decline to answer any questions, and to set boundaries around topics or practices that do not feel safe or comfortable for you.
  • Informed Participation: To ask questions about the process, methods, and practitioner’s qualifications, and to receive clear information about what to expect.
  • Feedback and Communication: To express concerns, provide feedback, or request changes in how sessions are conducted.
  • Pause or Stop: To pause or end a session at any time, for any reason, without explanation.
  • Access to Support: To request referrals or additional support if needed.

As your practitioner, I am committed to:

  • Honoring your boundaries and choices at all times.
  • Creating a safe, non-judgmental space for your healing journey.
  • Listening to your needs and adapting my approach to support your comfort and well-being.
  • Never pressuring you to share or participate beyond what feels right for you.

Waiver & Indemnification

Purpose of Session:

I, the undersigned, acknowledge that the services provided by Zodie Klempp or Edwin Hogendoorn and Promoveo Energy and Wellness are not a substitute for medical diagnosis, treatment, or advice from a licensed healthcare provider. I understand that all information shared is for educational and spiritual purposes only.

I agree to indemnify and hold harmless Zodie Klempp and Promoveo Energy and Wellness from any and all claims, liabilities, damages, or expenses arising from my participation in these services. I accept full responsibility for my own health and healing journey, and I enter into this contract with myself to seek hope, trust, and transformation, knowing that the practitioner will work on my behalf with rigor and respect.

Consent & Data Privacy

Collection and Use of Information:
I consent to the collection of my personal, health, and wellness information for the purpose of this session. My information will be used solely to support my care and will not be shared with third parties without my explicit permission, except as required by law.

Data Privacy and Security:
All information provided will be stored securely and handled in accordance with applicable privacy laws (PIPEDA). Electronic records are encrypted and access is restricted to authorized personnel only.
I understand that while every effort is made to protect my privacy, electronic communication (such as email or online forms) may carry some risk of unauthorized access.

My Rights:

  • I have the right to request access to my records and to correct any inaccuracies.
  • I may withdraw my consent at any time, understanding that this may affect my ability to receive services.
  • I may request that my information be deleted or transferred, subject to legal and professional requirements.

Confidentiality:

Trauma-Informed Consent Statement

Your comfort, safety, and autonomy are my highest priorities. The information you choose to share in this intake form and during our sessions is entirely voluntary. You are welcome to skip any questions or topics that do not feel safe or comfortable for you to answer.
